Swedish Medical Center is looking for a Clinical Social Worker (0.9 FTE, Night Shift) to work in Case Management at Swedish Mill Creek in Everett, WA.

SCHEDULE: This position will work three, 12 hour shifts per week (36 hours; 7pm-7:30am PST). Days to be worked rotate between Wednesday, Thursday, and Friday, OR Thursday, Friday, and Saturday each week.
The Social Worker is responsible for providing case coordination and a full range of social work services including psychosocial assessment, treatment planning, utilization management, therapeutic interventions, discharge planning, crisis intervention, referral and resource, and liaison with insurance providers. The primary focus is on patients in medical rehabilitation.
In this position you will:
Works collaboratively with patients nurses, case managers, physicians, other practitioners, caregivers and community resources and agencies. The social worker and healthcare team are jointly accountable for measurable outcomes that are cost effective and reflect patient preferences and values.
Communicates timely, relevant and accurate information to all parties involved with a patient’s care.
Facilitates the progression of care by advancing the care plan to achieve desired outcomes
Integrates the work of the healthcare team by coordinating resources and services necessary to accomplish agreed-upon goals
Advocates on behalf of patients and caregivers for service access or creation, and for the protection of the patient’s health, safety and rights.
Assures prudent utilization of all resources (fiscal, human, environmental, equipment and services) by evaluating the options available and balancing cost and quality to assure the optimal clinical and financial outcomes.
Required qualifications for this position include:
Master's Degree in Social Work from an accredited school
Licensed Social Work Associate – Independent Clinical (LSWAIC) required.
Licensure as an Independent Clinical Social Worker (LICSW) with the Washington State Department of Health under Chapter 18.225 RCW is preferred and may be substituted
One (1) year experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting
