Swedish Health Services Pharmacist Resident in Seattle, Washington

Description

The Pharmacy Practice Residency is a one-year post-graduate residency program in an institutional practice setting that provides training as set forth and approved by the American Society of Health-Systems Pharmacists (ASHP). The resident will acquire knowledge and experience beyond the traditional academic experience, undergraduate pharmacy clerkships and general practice hospital experience. The resident will actively participate in the development and implementation of department goals and objectives to improve pharmaceutical services and standards of patient care. The resident will promote department's delivery of pharmaceutical care with the primary focus on patient outcome.

  • Continuously involved in the development, implementation and monitoring of departmental operations with department managers.

  • Functions as a staff pharmacist (see Staff Pharmacist Job Description) every other weekend and as needed as deemed appropriate by the Pharmacy Directors.

  • Assures safe, effective, and cost-effective use of drugs on behalf of the patient through the application of specialized knowledge, skills and professional judgment while considering the patient’s disease condition(s).

  • Develops and maintains knowledge of age specific issues and applies that knowledge to the provision of care within the institution.

  • Provides and documents drug information for physicians, nurses and patients; utilizing specialized knowledge critical thinking and available drug resources.

  • Complies with formulary management and therapeutic substitution initiatives.

  • Assists in the development of protocols, guidelines and critical pathways in conjunction with staff and managers.

  • Assists in the reporting of Adverse Drug Reactions and Medication Errors with ideas and changes that foster the prevention of future incidents.

  • Assists the Clinical Manager and other staff in developing and maintaining quality assurance programs, new clinical services, an effective formulary, and appropriate policy and procedures.

  • Actively participates on the Pharmacy and Therapeutics Committee in maintaining and updating the formulary.

  • Assists preceptors with the supervision of pharmacy clerkship externs and student interns.

  • Participates in the training and development of pharmacists, technicians and other health care disciplines.

  • Provides direct supervision of pharmacy technicians by overseeing and monitoring their daily activities.

  • Participates as a member of multi-disciplinary and pharmacy committees as deemed appropriate by the Pharmacy Directors.

  • Maintains patient confidentiality standards as stated per Swedish policy.

  • Assures compliance with hospital and departmental policies, procedures and practice guidelines.

  • Assists the Pharmacy Directors in preparing the department for JCAHO and State Board of Pharmacy reviews.

  • Provides clinical current and relevant drug information to Swedish staff through contribution of articles for the Pharmacy Newsletter.

  • Completes BCLS training on an annual basis.

Required Qualifications:

  • PharmD - Pharmacy from an accredited college or university or equivalent educ/experience.

  • Washington Pharmacist Intern Registration upon hire -or-

  • Washington Pharmacist License upon hire.

  • Must have completed a comprehensive clerkship and externship program as part of the pharmacy curricular.
