Social Worker MSW - ED Navigator at Swedish Shared Services
SCHEDULE: (1.0 FTE) Monday - Friday 9am-5pm PST. No nights or weekends.
The ED Navigator is a Masters level Social Work position and provides intensive engagement, case-management, linkage and follow-up services for individuals who have been identified as Swedish Emergency Department's Most Visiting Patients (MVPs). This position reports to the Case Management ED MSW Manager and is responsible for identifying and referring MVPs for service coordination with State, County and community services. The ED Navigator provides assertive outreach and engagement services with MVPs and develops relationships with community providers to facilitate linkage and coordination with on-going services and community supports. The ED Navigator will be expected to be able to work with a wide variety of patients and be sensitive to the culture, religion, language, age, gender, and sexual orientation of our MVPs.
Master's Degree in Social Work
Washington Advanced Social Worker License upon hire -or-
Washington Advanced Social Worker Associate License upon hire -or-
Washington Clinical Independent Social Worker License upon hire -or-
Washington Clinical Independent Social Worker Associate License upon hire -or-
Washington Substance Use Disorder Professional License upon hire
3 years of direct delivery of professional mental health care inclusive of Case Management
1 year of Case Management experience with a behavioral health population
6 months of Social work in a medical setting, inpatient or outpatient
